Let me briefly describe the pathophysiology of vertigo

Vertigo is the feeling of internal or external spinning sensation which can occur due to many causes. The usual triggers to vertigo are rapid head movement, standing too quickly, prolonged head bending, foods like excessive salt, nicotine, caffeine, alcohol, etc. 

Vertigo can have some prodromal symptoms which warn us about the attack like dizziness, hearing loss, fullness in the ear, excessive sweating, vomiting sensation, unable to focus properly, blurry vision, ringing in the ears, nystagmus, loss of balance, etc. Vertigo can occur spontaneously or it can be associated with some other pathologies. The pathology can be Central Or Peripheral causes. 

What are the central causes of vertigo?

  1. Stroke 
  2. Transient Ischemic attack 
  3. Cerebellar infarct  
  4. Multiple Sclerosis 
  5. Vestibular migraine

Peripheral causes of vertigo 

  1. BPPV – Benign paroxysmal positional vertigo 
  2. Meniere’s disease 
  3. Cogan’s syndrome 
  4. Acoustic neuroma 
  5. Vestibular neuritis 
  6. Ramsay hunt syndrome 
  7. Any disease of the inner ear which can distort balance 

BPPV is the most common cause of vertigo which occurs due to the abnormal movement of otoliths in the ear.  It can last for a few seconds to a few minutes. It completely resolves on its own, unless there is some serious pathology.

Treatment of vertigo 

  1. Immediately lying down and maintaining the same position without head movement. 
  2. Adequate rest without head movement. 
  3. Epley’s manoeuvre. 
  4. Medicines like chlorperazine, Vertin is most commonly used.
  5. If the condition doesn’t improve immediately consult the physician.

Many people suffer from dizziness which has a similar presentation to vertigo, but vertigo can be a warning sign for serious problems.
